Supporting CN through his mental health struggles
PAMAN medication monitoring helps to achieve the best health outcomes for our Service Users.
Background
CN has a history of drug and alcohol misuse and struggles with mental health issues. His mobility challenges have led to frequent falls, adding to his health complexities. He lives close to his sister, who offers support, and spends a lot of time at her home.
To assist with medication management, the PAMAN team installed a REMLOK, a remotely monitored medication cabinet, allowing CN access to his medication only at with PAMAN monitors at scheduled times. This system helps ensure safe medication adherence.
Overcoming challenges
CN experienced frequent delays in accessing his repeat prescriptions, which led to missed doses. This inconsistency posed a risk of mental health relapse, as he needs to take his medication regularly to maintain stability.
The PAMAN team coordinated with his pharmacy to have prescriptions dispensed and delivered at least 48 hours in advance. Despite these efforts, CN sometimes ran out of medication and only received his repeat medications late in the day, resulting in doses being taken too close together, raising concerns within the PAMAN team of possible overdose.
Improving Health Outcomes
The PAMAN team escalated the issue to CN’s GP, who arranged for prescriptions to be pre-dated to ensure timely availability. With a stable medication routine now in place, CN has shown significant improvement, allowing his GP to reduce his Methadone dosage. Both the PAMAN team and CN’s GP are pleased with his progress.
