Supporting with Medication Management and Mobility Challenges

Background
VR was an active individual, however, following a car accident last year that injured her foot and worsened her severe arthritis, she can no longer drive and now relies on others for transportation. Additionally, she has been diagnosed with memory decline and referred to a memory clinic for further assessment. This loss of independence has been challenging for VR, affecting her confidence and routine.
Challenges
VR’s daughter initially managed her medication, dispensing it into a weekly dosette box. However, concerns arose when VR accidentally took two days’ worth of medication at once. This, combined with her increasing memory issues, prompted her daughter to request assistance from the PAMAN team to help maintain medication compliance and ensure safety.
Outcome
Upon referral from VR’s GP, the PAMAN team performed a comprehensive medication check, removing outdated and discontinued items and addressing overstocked medications. VR’s confusion with her medication regimen was evident, she was taking her Spiriva inhaler twice daily instead of once, which the PAMAN team corrected. During calls, monitors worked closely with VR, guiding her with picking up the correct tablets, ensuring all medications were taken correctly by the end of each call.
To simplify her medication routine, the team arranged for a blister pack system and took over ordering her repeat prescriptions, there had been several instances of running out of medication. VR expressed her gratitude, noting that the PAMAN team’s support feels like having her own personal nurses, which has significantly eased her daily routine and provided reassurance for both her and her family.
